Corrections and review policy
Material factual changes are dated, source linked and preserved in the affected record’s history.
How corrections work
- A reported issue is triaged against the source hierarchy.
- The current regulatory or clinical source is checked by a reviewer.
- If material, the previous wording, corrected wording, date and source are recorded.
- The record version and last verified date are updated.
Review cycle
Priority records are reviewed after material regulator, NICE, NHS, trial registry or sponsor events and on a scheduled periodic cycle. A review date does not mean every source changed on that date.
Report a concern
Use the professional beta form and select the corrections purpose in your message. Include the affected record, disputed wording and the strongest source you believe should be considered.
