Corrections and review policy

Material factual changes are dated, source linked and preserved in the affected record’s history.

How corrections work

  1. A reported issue is triaged against the source hierarchy.
  2. The current regulatory or clinical source is checked by a reviewer.
  3. If material, the previous wording, corrected wording, date and source are recorded.
  4. The record version and last verified date are updated.

Review cycle

Priority records are reviewed after material regulator, NICE, NHS, trial registry or sponsor events and on a scheduled periodic cycle. A review date does not mean every source changed on that date.
